Survey responses were collected from 1,067 active Project FoodBox members through a standardized effectiveness questionnaire administered in August 2025. Measures included dietary behaviors, symptom frequency, program engagement, and validated well-being scales. The WHO-5 Well-Being Index is a widely used short-form measure of emotional and psychological well-being consisting of five questions (e.g., feeling cheerful, calm, rested). Each item is scored 0–5, summed, and multiplied by 4 to yield a score from 0 (worst possible well-being) to 100 (best). Higher scores reflect greater overall well-being and are predictive of both quality of life and reduced risk of depression.
