Clients receive weekly deliveries of food tailored to their health conditions. Depending on plan and clinical needs, they may receive Medically Supportive Food (MSF), Medically Tailored Groceries (MTG), or Medically Tailored Meals (MTM). Box type is determined at enrollment based on the client's health plan authorization and clinical profile, reviewed by a Registered Dietitian.

Box contents vary by modality (MSF, MTG, or MTM), health plan contract, and seasonal availability. MSF and MTG boxes contain approximately 15–18 lbs of fresh produce and shelf-stable pantry items sourced locally where possible. MTM boxes contain pre-frozen, portioned meals from partner kitchens. All boxes are curated by Registered Dietitian Nutritionists and updated seasonally.

Project FoodBox delivers three distinct modalities determined by the member's clinical profile, plan contract, and condition-specific nutritional requirements: Medically Supportive Food (MSF) provides fresh produce aligned with national dietary guidelines; Medically Tailored Groceries (MTG) provide condition-specific produce and pantry items curated by a Registered Dietitian Nutritionist; Medically Tailored Meals (MTM) provide pre-portioned frozen meals from partner kitchens designed to meet condition-specific targets. Modality selection is determined at enrollment by the RDN in coordination with the health plan benefit authorization.

Clients must be enrolled in a participating Medicaid or Medi-Cal health plan and have a qualifying health condition. Eligibility varies by state. In California, qualifying conditions include diabetes, heart disease, high blood pressure, chronic kidney disease, and obesity. In New York, eligibility includes all California-covered conditions plus pregnancy, recent unemployment, and general food insecurity. If you are unsure whether a client qualifies, contact us and our team can run an eligibility check.

No. The program is fully covered by the member's Medicaid or Medi-Cal health plan. There are no fees for members. This is a benefit, not a service they pay for.

Contact our partner team to set up a referral workflow that fits your organization. Referrals can be submitted individually or in batches, depending on volume. Our team handles eligibility verification—you do not need to confirm plan participation before submitting. Not necessarily. Depending on the client's health plan, a physician referral may or may not be required. In many cases, a referral from a social worker, care coordinator, or community health worker is sufficient. When you submit a referral, our team will determine what documentation is needed based on the client's specific plan.

At a minimum, we typically need the client's full name and date of birth, health plan name, and a qualifying condition or clinical note when required by the plan. Organizations with an active data-sharing or referral agreement in place can submit structured referral files. Contact our partner team to confirm the intake format that works for your workflow.

If your organization works with individuals managing chronic conditions who are enrolled in Medicaid or Medi-Cal, we want to hear from you. In some markets, yes. Project FoodBox primarily delivers directly to members' homes, but we are open to exploring community distribution models with organizations that serve a high concentration of eligible members. Contact our partner team to discuss what is feasible in your service area.
