How do you qualify?
To qualify, you must demonstrate ongoing need for nutritious, medically tailored meals. Our team reviews eligibility to ensure you receive fresh, high-quality produce that supports your health goals. We’re here to make healthy eating accessible to you and your community.
Enrolled in Medicaid Managed Care
- Receive coverage through state-contracted managed care organizations (MCOs). These programs coordinate care, improve quality, control costs, and provide benefits like disease management and care coordination.
Meet Enhanced Population Criteria
- Eligibility includes people with chronic illnesses (such as diabetes, heart failure, or kidney disease), older adults, persons with disabilities, or others classified as high-risk by the state.
State-standardized HRSN Screening
- Must have at least one unmet health-related social need (such as food insecurity or housing instability), confirmed using the Health-Related Social Needs (HRSN) Assessment tool issued by your provider.
Additional Clinical or Social Risk Factors
- For nutrition support, there must be a documented medical need for dietary help (such as medically tailored meals for chronic conditions or disease management).

Patients are referred to Project FoodBox by their healthcare provider or plan. Once enrolled, eligible individuals receive regular deliveries of fresh produce and other nutritious items tailored to their specific health needs.
Eligibility depends on your health insurance provider, care plan, and medical condition. Typically, individuals managing chronic illnesses like diabetes, heart disease, or hypertension may qualify through a Medi-Cal CalAIM referral.
"Medically tailored" refers to food that is selected and portioned based on a person’s specific health condition, dietary restrictions, and nutritional needs — all designed to support clinical outcomes and overall wellness.
Each box includes a variety of fresh produce, whole grains, and other nutrient-rich items. The selection may vary based on the season and availability, but all items are chosen with health and freshness in mind.
