Assess Need
Medi-Cal Members contact PFB (by referral or directly) are virtually assessed and enrolled for up to 24 weeks.
Tailor Meals
Registered Nutritionists (RDN) use evidence-based practices to tailor each 15 – 18 lb. box to meet individual’s unique health needs.
Source Nutrition
A robust supply-chain ecosystem sources, packs, and ships nutritious food from local farms to doorstep in 48 hours adhering to FDA and HIPAA standards.
Delivered Wellness
A weekly medically-tailored grocery box is delivered directly to participant’s home.

Am I eligible for the program?
You may be eligible if you are a Medi-Cal member residing in Fresno, Imperial, Los Angeles, Mariposa, Merced, Monterey, San Benito, Santa Cruz, and Orange Counties and have a qualifying health condition. See the complete list of qualifying conditions above.
What kind of food will I receive?
Our boxes contain a variety of seasonal fruits and vegetables chosen to be both delicious and nutritious. Registered Nutritionists (RDN) selects and customizes the food to fit your dietary requirements.
How often will I receive deliveries?
Once per week – Up to 24 weeks.
